What does a Support Coordinator do?
A Support Coordinator:
- Works with you to determine what NDIS funding you have available for services.
- Assists you to identify what you would like to get out of your services and how, when and where you would like these delivered.
- Whenever possible, supply you with a range of potential service providers, to enable you to make an informed choice.
- Assist you to identify service providers in your area that are most likely to meet your needs.
What are the different types of Support Coordination at Assured Care Solutions?

We will assist you in setting up your services, continuously monitor and manage their progress against your stated goals, and step in when needed to help resolve any service-related issues.
Specialist Support Coordination
Our qualified practitioners provide specialist advice and support for individuals with high-level or complex needs, as recognised and funded in the participant’s NDIS Plan. We will help you address any challenges or barriers you may encounter and ensure you are maximising the full benefits of your Plan.
How do I know if I'm eligible to receive funded Support Coordination services?
Under the NDIS, Support Coordination is known as Capacity Building. It will be listed in your NDIS plan under this area. Eligibility for NDIS funding for support coordination is determined at your NDIS Plan Review meeting.
Can my Assured Care Support Coordinator assist me with my NDIS Plan Review meeting?
Yes. Your Assured Care Support Coordinator can assist you in a number of ways including:
- Preparation for the NDIS Plan Review meeting
- Attending the NDIS Plan Review meeting with you on the day and
- After your NDIS Plan Review meeting to assist with any follow up required.
If I have an existing Support Coordinator, can I change before my plan is finished
Yes. All you need to do is contact your existing Support Coordinator to tell them you want to end the arrangement with them and then contact us and we will arrange the rest. Please note all providers will have different notice periods and this can usually be found in your service agreement.
